Due to unprecedented growth moving into 2020, Hospice Home Care (HHC) has an opening for a Licensed Social Worker based out of our Hot Springs Branch Office.

As a Social Worker, you will serve as a critical advocate for each of your assigned patients and their families. Hospice Home Care’s Social Workers have built a strong reputation as some of the most competent and impactful professionals in the Hospice industry. This is an extremely important role within our organization and one that helps distinguish us by providing the absolute best patient care.

Primary day to day duties include performing initial/ongoing assessments and assisting in the development and implementation of individualized plans of care. Additionally, you’ll serve as the primary link between HHC patients and all appropriate social resources available within the community that foster quality of life.

Additional Duties and Responsibilities:

• Provide short term crisis intervention, along with individual/family counseling as needed - this may include mediating issues that arise among patients, families, and caregivers.
• Actively participates in team meetings to ensure the patient’s changing needs are being addressed and met.
• Document accurate and ongoing assessment of patient status via a variety of mediums of communications (i.e. verbal, written, computer software/company technology)

Requirements:

• Bachelor’s Degree in Social work from an accredited University (candidates with Master’s level degrees highly desired)
• Active Social Work License in Arkansas
• Previous experience with Patient Care
